The Relationship of the Neurological Status and A ngiographical Evidence of Spasm to Prognosis in Patients With Ruptured Intracranial Saccular Aneurysms • A retrospective analysis of 79 patients admitted to the Neurosurgery Department, Rigshospitalet, Copenhagen, Denmark, from January, 1969, through May, 1971, was performed. The clinical grading was performed according to a modification of the scheme of Hunt and Hess. All patients were evaluated with angiography. Twenty-five of the 79 patients had preoperative angiographical evidence of spasm. Surgery was delayed for a mean period of 15 days in the majority of patients except for those with significant mass effects. Sixty-two of the 79 patients reached operation. Fortysix of the 62 patients had angiography performed immediately upon completion of surgery. Twenty-two of the 46 patients had angiographical evidence of spasm immediately postoperatively. The eventual outcome of all of the patients is evaluated according to the clinical grade and the presence of spasm preoperatively and postoperatively. The high incidence of spasm immediately after surgery shows that spasm is not a delayed phenomenon as is suggested by clinical evaluation. Methods for treating spasm should be started at the time of surgery. Additional

CSF serotonin concentrations and cerebral arterial spasm in patients with ruptured intracranial aneurysm.

In 26 patients with recent rupture of an intracranial saccular aneurysm the CSF concentrations of serotonin (5-HT) were measured repeatedly by a radioimmunoassay. The 5-HT level in ventricular CSF collected between the 2nd and 15th day after SAH ranged between less than 2 and 5 nmol/l. These did not differ from the levels found in the ventricular CSF (less than 2-3 nmol/l) and lumbar CSF (less ...

Serum Levels of Monocyte Chemoattractant Protein-1 Correlate with Poor Clinical Grades in Cerebral Aneurysms

Background : Ruptured cerebral aneurysms (ICAs) are the most common non-traumatic cause of subarachnoid hemorrhage (SAH) that is associated with life threatening complications such as Vasospasm, Infarction, and Hydrocephalus (HCP). The active participation of macrophage/monocyte-mediated inflammatory response in the pathogenesis of cerebral aneurysm as labeled with Monocyte ChemoattractantProte...

Risk of Dependency Following Microsurgical Clipping in Good Grade Patients with Ruptured Anterior Circulation Aneurysms

Background: It is well known that the majority of good grade (WFNS I and II) patients who undergo microsurgical clipping achieve a favorable outcome, but still a number of patients who are independent before surgery face unfavorable outcomes signifying the impact of microsurgical clipping. The aim of this study was to quantify the risk of dependency in patients who had no previous neurological ...

Successful Coil Embolization of a Ruptured Basilar Artery Aneurysm in a Child with Leukemia: A Case Report

Ruptured intracranial aneurysms are rare in the pediatric population compared to adults. This has incited considerable discussion on how to treat children with this condition. Here, we report a child with a ruptured saccular basilar artery aneurysm that was successfully treated with coil embolization. A 12-year-old boy with acute lymphoblastic leukemia and accompanying abdominal candidiasis aft...
